• The name of the account holder on your Electric Ireland account (found on Electric Ireland bill)
• Your Electric Ireland account number (found on Electric Ireland bill)
• MPRN (found on Electric Ireland bill)
• Meter Number (found on Electric Ireland bill)
• Contact Name
• Contact telephone number
• Meter reading (found on your meter)

If you have more than one meter of the same type on the same account, submit the form as many times as necessary.

For example, if you have two Standard Meters, you must enter and submit the Meter Reading for each by using the form twice.
